MyDiabPal. Co-creating and testing an illustrated self-management support package for person-centred diabetes care in interdisciplinary primary care services.

The overall objective of the MYDIABPal project is to empower T2DM diabetes and prediabetes
patients towards therapy adherence with an integrated support package for motivation and
diabetes self-management (DSM). MyDIABPal will develop and evaluate by co-creation and a
pilot study three tools: (1) an illustrated DSM tool (playbook), (2) adjacent workshops for
patients and (3) a DSM training for healthcare providers. The project will be implemented in
three stages spread over three years.
